I vote for #3, we had a similar incident in our ED with a woman who wouldn't let her daughter be seen by a PA, a Med Student, a resident, or an intern. It had to be an attending. Well on most days we have only one or two attendings and several PAs, residents, and interns. It happened to be that we had one attending that day, and he decided to let the woman sit and wait for 4 hours. He sent in someone every half hour or so and she would send them back out and say her daughter had to be seen by an attending. Finally after 4 hours he went in and saw her, turned out she had a UTI... heh!

I should say that this was a slow day and we had no need for the room to be empty, so we just kept charting "pt refuses to be seen by anyone except the attending; attending with other pts." And so on and so forth for 4 hours.

I bonk my head every single day over dealing with people such as this!
