The skeleton should not be there, as this is an X-ray of an earthworm.
That sure looks like a fist on the “L” side. But if it were real, we’d be seeing fingerbones, not the outline of the skin. So I’m guessing Jeff is right, that’s a fist-shaped dildo in the descending colon.

Looks like an unfortunately shaped small bowel obstruction
plastic obstruction, I am guessing in the colon. Otherwise I have no idea how something that looked like that got in there.
